Persons Age: I don't need the exact age unless you want to tell me. I just need a general time
frame for the poem.

Sex: Male or Female

Physical Description: Do they have any distinguishing features: very blue eyes, tall, great
smile, cheerful laugh. Anything that would describe a memorable characteristic.

Hobbies: What pastimes do they like to do: gardening, sailing, fishing, sports, sewing, golf.
Things that you always see them doing that they enjoy.

Friends: What kind of friend are they? Are they considerate, compassionate, giving, caring,
thoughtful, loving, kind, courageous, brave, willing to give a hand.

Workplace: Are there specific details about where they have worked and their
accomplishments? Did they start out in one career and change to something they
really enjoy?

Personality: List some qualities that you would like to share? Do they always make people
laugh, are they a good listener, are they a good cook who loves to share their feast? Are they
a loving parent or grandparent always willing to teach and share life's experiences?

Goals and Dreams: Is there something they accomplished in their life that they are very proud
of? Did they acquire fame and fortune at an early age? Do they dream of scaling new heights
in either a sport or the entertainment fields?

Life's Travels: Is there anything significant about their life's path: where they grew up, the town
they lived in, who they met along the way.

Family Interaction: Do they belong to a large or small family? As a family do they do anything
special together. Are summer vacations spent in interesting places? Are they musically or
artistically inclined? Are there any family traditions you need to mention?

Special Memories: Describe any special memories that they were a part of school,
college, or church.

Relationship With God: Are they prominent in their church? Are they mission oriented or
evangelistic? Do they have a prayer life (daily devotion, bible studies and time alone with God?

Someone Special In Their Lives: Is there someone special in their lives that was/is a mentor or
teacher? Is there a person they have patterned their life after?
